Physical Therapist (PT)

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility/ROM, transfer status, balance, strength.
• Implement integumentary protective interventions to prevent wounds and promote healing.
• Develop and implement individualized care plans, including manual exercises, gait training, balance.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• A current NJ Physical Therapist - PT license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program or AMA CAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
