Physical Therapist (PT)

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, gait, strength, balance.
• Implement wound care and protective interventions.
• Develop and implement individualized care plans with manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with nursing.
• Educate clients and families in rehabilitative care for health and safety.
• Accurately document observations and interventions using a touch pad tablet/EMR.

🎯 Requirements

• A current NJ Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduate from an APTA/CAPTE/AMA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem solving skills.
• Solid computer skills; EMR experience preferred.
