Visit clients in their homes within designated territories
Perform diagnostic tests and measurements (mobility, gait, balance, strength)
Implement wound care and protective interventions as indicated
Develop and revise individualized PT care plans; include manual therapy and gait/balance training
Educate clients and families to promote health, safety, and independence
Accurately document observations and interventions using a tablet

🎯 Requirements

Active and current PT license
One year of clinical experience preferred
Home health care experience welcomed, but not required; training provided
Ability to travel locally to cases
