Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility, ROM, gait, balance, and strength.
• Implement integumentary protective interventions for wound prevention and repair.
• Develop and implement individualized PT care plans, including manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and family members in rehabilitative care to promote health and independence.

🎯 Requirements

• A current North Carolina Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an APTA-approved program; indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
