Physical Therapist, PT - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, strength, balance, safety).
• Implement wound care and protective interventions to prevent wounds and promote healing.
• Develop and implement individualized care plans (manual therapy, gait, balance).
• Continually assess and revise the PT care plan with nursing and multidisciplinary teams.
• Educate clients, family members, and caregivers in rehabilitative care and independent living.

🎯 Requirements

• Current NC Physical Therapist license (PT).
• Minimum 1 year PT experience.
• Graduation from APTA/CAAHEA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
