Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform tests: mobility, ROM, balance, gait, strength.
• Implement wound care and dressings as needed.
• Develop individualized care plans with manual therapy and gait training.
• Assess and revise PT plan; coordinate with nursing.
• Educate clients and families on home rehabilitation.

🎯 Requirements

• Current North Carolina Physical Therapist (PT) license.
• Minimum one year of PT experience.
• Graduate from APTA/CAHEA-AMA accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
