📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility and strength assessments; monitor ambulation and safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop individualized care plans including manual therapy and gait/balance training.
• Educate clients and families in rehabilitative care and independence.
• Document observations and care using a touch-pad tablet.

🎯 Requirements

• North Carolina Physical Therapist license.
• Minimum 1 year PT experience.
• Graduate from APTA/CAAHEA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
