Physical Therapist, PT, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, gait, strength, balance, safety).
• Implement protective interventions to prevent wounds and promote healing.
• Develop and implement individualized care plans (manual therapy, gait/balance training).
• Continuously assess and revise PT care plans; collaborate with nursing.
• Educate clients and families on rehabilitative care and safety for independent living.

🎯 Requirements

• A current OH Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduate from an APTA-approved program or AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
