Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfer status, ambulation, strength
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and implement individualized care plans: manual therapy, gait training, balance, other
• Continuously assess and revise PT care plan; collaborate with nursing in multidisciplinary plan.
• Educate clients and family members in rehabilitative care and safety for independence.

🎯 Requirements

• A current North Carolina Physical Therapist license.
• Graduate from APTA-approved program (or AMA accred); evidence in transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
