Physical Therapist (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (e.g., mobility/range of joints, transfer status
• Implement integumentary protective interventions to prevent wounds and promote wound repair
• Develop and implement individualized care plans including manual therapy, gait training, balance
• Continually assess and revise PT care plan, participate in multidisciplinary care plan
• Educate clients, family members, or representatives in rehabilitative care and activities

🎯 Requirements

• Current North Carolina Physical Therapist license
• Minimum of one year recent work experience as a Physical Therapist
• Graduation from an APTA-approved or Committee on Allied Health Education and Accreditation approved
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; prior experience with EMR preferred
