Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: mobility, ROM, balance, safety.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and implement individualized care plans (manual therapy, gait, balance).
• Continually assess and revise the PT care plan; coordinate with nursing in a multidisciplinary plan.
• Educate clients and families in rehabilitative care to promote health, safety, and independent living; document observations and interventions in EMR.

🎯 Requirements

• Current PA Physical Therapist (PT) license.
• Minimum 1 year of PT experience.
• Graduate from an APTA-approved program or AMA CAAHEA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior EMR experience preferred.
