Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Apply wound care using evidence-based dressings to prevent and repair wounds.
• Develop and implement individualized PT plans with exercises, gait, and balance.
• Assess and revise PT plans; collaborate with nursing in MDT.
• Educate clients and families in rehabilitative care for health and independence.

🎯 Requirements

• Current OH PT license.
• Minimum of 1 year PT experience.
• Graduation from an APTA/CAHEA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
