Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Conduct PT assessments: mobility, ROM, strength, balance, safety.
• Provide wound care interventions using evidence-based dressings.
• Develop individualized care plans: manual therapy, gait, balance.
• Continually assess and revise the PT care plan with nursing.
• Educate clients and family on rehabilitative care for independence.

🎯 Requirements

• PA PT license.
• Minimum 1 year PT experience.
• Graduate from APTA-approved PT program.
• Able to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with computers; EMR experience preferred.
