Physical Therapist, PT Outpatient Visits

📋 Description

• Make outpatient visits to clients in designated senior living communities.
• Perform mobility assessments, ROM, transfers, balance, and safety.
• Develop and implement individualized PT care plans, including gait training.
• Assess and revise the PT plan of care; participate in care plan meetings.
• Educate clients and families in therapy services to promote independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Licensed Physical Therapist in Massachusetts.
• Minimum 1 year of clinical PT experience.
• Interest in geriatrics and aging in place; patient-centered care.
• Ability to work with multidisciplinary teams in home health.
