Physical Therapist, PT - Senior Living

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, strength, balance, and safety.
• Provide wound care with evidence-based dressings.
• Develop and adjust individualized PT care plans.
• Collaborate with nursing on multidisciplinary care plans.
• Educate clients and families on rehab activities.

🎯 Requirements

• North Carolina PT license.
• Minimum of one year PT experience.
• Graduation from APTA/CAAHEA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
