📋 Description

• Make home visits to clients in designated geographic territories.
• Perform tests and measurements: mobility, ROM, stability, gait, strength, balance.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop care plans including manual therapy, gait training, balance, and other interventions.
• Assess and revise PT care plan; coordinate with nursing.
• Document observations and interventions using EMR.

🎯 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of PT experience.
• Graduate from an APTA-approved program or AMA CAAHEA.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
