Physical Therapist - Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfer, balance, strength, safety.
• Apply wound-preventive care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait and balance training.
• Continuously assess and revise the PT care plan; coordinate with nursing.
• Educate clients and families on rehabilitative care to promote health and independence.
• Document care observations and interventions on a touch-pad tablet.

🎯 Requirements

• Maryland PT license.
• Minimum 1 year PT experience.
• Graduate from APTA/CAAHEA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills with EMR experience preferred.
