Physical Therapist - Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfers, balance, strength, safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans: manual therapy, gait and balance training.
• Continually assess and revise PT care plan with nursing.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• A current Maryland Physical Therapist license (PT).
• A minimum of one year of recent work experience as a Physical Therapist (PT).
• Graduation from an APTA-approved program (per transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
