Physical Therapist - Senior Living Visits

📋 Description

• Make home visits to clients in designated territories.
• Conduct mobility, strength, balance, gait, and safety assessments.
• Provide wound care with evidence-based dressings and topical agents.
• Develop and implement individualized PT care plans (manual therapy, gait/balance).
• Assess and revise the PT care plan with nursing on the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety.

🎯 Requirements

• Maryland Physical Therapist license.
• Minimum 1 year PT experience.
• Graduate from an APTA-approved PT program or AMA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Strong computer skills; EMR experience preferred.
