Physical Therapist - Weekend SOC Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests like mobility, strength, balance, and safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and implement individualized care plans: exercises, gait, balance.
• Assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients and families on rehab and independent living.

🎯 Requirements

• A current Maryland Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduate from an APTA-approved program as shown on transcript.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
