Provide home health PT visits to adult and geriatric patients.
Deliver treatment, tests, education, and interventions as directed by a licensed PT.
Perform per-visit home health visits in the Williamsburg to Hampton, VA area.
Coordinate care with a licensed PT.
Maintain safety and accurate documentation per BAYADA policies.

🎯 Requirements

Current VA licensure/certification.
2-year APTA-approved PTA program with transcript or diploma.
One year PTA experience with proficiency exam completed before 12/31/1977.
At least 1 year under supervision of a PT.
Other activities as requested.
