📋 Description

• Develop and implement nutritional plan of care for participants that takes into account social
• Plan and execute follow ups and reassessment for each participant, or as necessary if there are
• Assess participants’ access to food and makes referrals to interdisciplinary team for meal
• Determine participants’ need for and order oral nutritional supplements accordingly.
• Provide instruction to participants and caregivers regarding dietary regimens and nutritional
• Develop and implement the nutrition care plans for high-risk participants including medical

🎯 Requirements

• Master’s degree or equivalent in food science, nutrition, and/or dietetics as required by the
• Current medical terminology and the ability to complete written assessments and document
• Nationally Certified as a Registered Dietitian Nutritionist.
• A minimum of one year experience working with the frail or elderly.
