📋 Description

• Visiting clients at home in assigned areas.
• Plan and conduct dietetic education for staff, clients, and caregivers.
• Assess and counsel on medical nutrition therapy for prevention and disease management.
• Provide reliable information and education in home health care.
• Translate science into practical guidance for staff and families.
• Document care in EMR and collaborate with the home health team.

🎯 Requirements

• Bachelor's degree in Food and Nutrition from an accredited program.
• Active membership and registration in the American Dietetic Association.
• Licensed Registered Dietitian in PA.
• Minimum 2 years of RD experience in healthcare.
• Home health care experience a plus.
• Proficient with EMR and computer skills; travel ability.
