Provide rehabilitative care and skilled nursing at home as part of a team
Follow designated care plans per patient needs
Make home visits to clients in designated territories
Administer medications, wound care, and treatments
Monitor patient conditions and report changes to the Clinical/Client Services Manager
Document doctor orders and perform client assessments

🎯 Requirements

Current RN license in SC
Minimum 1 year clinical (medical/surgical) experience
Graduation from an accredited nursing program
Ability to work independently and manage time
Strong interpersonal skills
Solid computer skills; EMR experience preferred
