📋 Description

• Provide skilled nursing and rehabilitative care at home.
• Perform head-to-toe assessments; detect subtle changes.
• Manage acute and post-acute care (CHF, COPD, wounds, IV).
• Triage and independently manage urgent clinical situations in the home.
• Medication reconciliation, patient teaching, and safety monitoring.
• Wound assessment, care, and documentation.

🎯 Requirements

• Current New Jersey nursing license in good standing.
• Graduation from an accredited nursing program (transcript or diploma).
• At least one year of adult/geriatric care experience in relevant settings.
