📋 Description

• Perform home health visits per patient care plan.
• Travel to patients in designated Salem County territories.
• Administer medications, wound care, and treatments.
• Monitor patient status; report changes to Clinical/Client Services Manager.
• Document orders and care using an EMR tablet.
• Conduct client assessments and coordinate care plans.

🎯 Requirements

• Current NJ RN license.
• Minimum 1 year clinical (medical/surgical) experience.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
