📋 Description

• Follow a designated care plan for patients at home.
• Make home visits to clients in Salem County, NJ.
• Administer medications, wound care, and treatments per plan.
• Monitor client conditions and report changes to supervisor.
• Document orders and care notes accurately (EMR).
• Conduct client assessments and coordinate case management.

🎯 Requirements

• A current NJ RN license.
• Minimum of one year of clinical (medical/surgical) experience.
• Graduation from an accredited nursing program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
