📋 Description

• Follow designated care plan per patient needs.
• Conduct home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to the supervisor.
• Document doctors' orders and perform client assessments as needed.
• Case management and coordination; document observations with a touchpad tablet.

🎯 Requirements

• Current NJ Registered Nurse license.
• Minimum of one year of recent clinical (medical/surgical) experience.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
