📋 Description

• Provide rehabilitative care and skilled nursing to home clients.
• Follow designated care plans and conduct home visits.
• Administer medications and perform wound care and treatments.
• Monitor client conditions and report changes to management.
• Document orders and complete client assessments.
• Coordinate case management and EMR documentation.

🎯 Requirements

• Current NJ RN license.
• Minimum one year clinical experience.
• Prior home care experience preferred.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time.
• Solid computer skills; EMR experience preferred.
