📋 Description

• Provide one-on-one skilled nursing visits for adults and geriatrics in senior living communities in
• Assess client needs and develop individualized care plans.
• Administer medications and perform wound care and physician-ordered treatments.
• Monitor client conditions and report changes to management.
• Coordinate care with physicians, therapists, and the interdisciplinary team.
• Educate clients and caregivers to promote health and independence.

🎯 Requirements

• Current New Jersey RN license.
• Minimum one year of recent clinical nursing experience.
• Home health or senior living experience preferred, but not required.
• Strong communication and organizational skills; EMR experience preferred.
• Reliable transportation to travel across Somerset, Middlesex, Monmouth, and Union Counties.
