📋 Description

• Part-Time home health visits for Gloucester County office, NJ
• Integral member of a multi-disciplinary team providing rehabilitative care and skilled nursing at
• Follow designated care plans and monitor patient conditions
• Travel to cases as assigned within geographic territories
• Document observations, interventions, and evaluations in EMR on a touch pad tablet

🎯 Requirements

• A current NJ Registered Nurse license
• Minimum one year of recent clinical (medical/surgical) experience
• Prior home care experience preferred
• Graduate of an accredited nursing program with transcript/diploma
• Ability to work independently and manage time effectively
• Strong interpersonal skills; solid computer skills; EMR experience preferred
