📋 Description

• Provide skilled nursing and rehabilitative care to home health clients as part of a
• Perform comprehensive head-to-toe assessments and identify changes in patient condition.
• Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and independently manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Complete wound assessments, care, and documentation in the electronic record.

🎯 Requirements

• Current nursing license in good standing in the state of Pennsylvania.
• Graduation from an accredited nursing program (transcript or diploma required).
• At least 1 year of hands-on medical experience with adult/geriatric populations in clinical
