Registered Nurse Case Manager - Home Health Visits

📋 Description

• Use head-to-toe assessment to identify subtle changes in patient condition
• Provide acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

🎯 Requirements

• Current Maryland nursing license in good standing
• Graduation from an accredited nursing program (transcript or diploma)
• At least one year of hands-on adult/geriatric care experience in acute care, home health, hospice, ICU, or rehab in past two years
• Ability to work independently and escalate concerns as needed
• Comfort with real-time clinical decisions without on-site support
