Registered Nurse Case Manager - Home Health Visits

📋 Description

• Head-to-toe assessment to identify subtle changes in patient condition
• Experience with acute and post-acute care (CHF, COPD, wound care, IV therapy)
• Triage and manage urgent clinical situations independently in the home
• Medication reconciliation, teaching, and safety monitoring
• Comprehensive wound assessment, care, and documentation

🎯 Requirements

• A current nursing license in Maryland
• Graduation from an accredited nursing program
• At least 1 year of hands-on experience with adult/geriatric patients in clinical settings
