Registered Nurse, Evenings

📋 Description

• Respond to after-hours patient needs within your assigned coverage area
• Complete Start of Care (SOC) admissions as needed
• Perform comprehensive patient assessments and complete OASIS documentation
• Deliver skilled nursing care, including medication administration and wound care
• Monitor patient conditions and communicate changes to the Clinical/Client Services Manager
• Coordinate care with physicians, caregivers, and the interdisciplinary team

🎯 Requirements

• Current Registered Nurse (RN) license in the State of New Jersey
• Minimum of one year of recent, verifiable Home Health nursing experience
• Experience completing OASIS documentation and SOC visits required
• Strong assessment, clinical decision-making, and communication skills
• Comfortable using EMR; Homecare Homebase (HCHB) experience is a plus
• Reliable transportation and ability to travel throughout Somerset, Union, Middlesex, and Monmouth Counties
