Registered Nurse - Evenings

📋 Description

• Respond to after-hours patient needs within your assigned coverage area.
• Complete Start of Care (SOC) admissions as needed.
• Perform comprehensive patient assessments and OASIS documentation.
• Deliver skilled nursing care, including medication administration and wound care.
• Monitor patient conditions and communicate changes to the Clinical/Client Services Manager.
• Document all patient care using an EMR system.

🎯 Requirements

• Current Registered Nurse (RN) license in the State of New Jersey.
• Minimum of one year of recent, verifiable Home Health nursing experience.
• Experience completing OASIS documentation and Start of Care visits required.
• Strong assessment, clinical decision-making, and communication skills.
• Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus.
• Reliable transportation and ability to travel throughout Somerset, Union, Middlesex, and Monmouth Counties.
