Registered Nurse, Evenings

📋 Description

• Provide evening in-home skilled nursing visits for adults and geriatric patients.
• Complete SOC admissions as needed.
• Deliver skilled nursing care, including meds administration and wound care.
• Conduct comprehensive assessments and EMR documentation.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• On-call during scheduled hours; respond to after-hours needs.

🎯 Requirements

• Current RN license in the State of New Jersey.
• Minimum 1 year home health nursing experience.
• Experience completing OASIS documentation and SOC visits.
• Strong assessment, clinical decision-making, and communication skills.
• Comfortable using EMR; Homecare Homebase (HCHB) a plus.
• Reliable transportation; ability to travel across Somerset, Union, Middlesex, and Monmouth Counties.
