Make home visits to clients in assigned territories
Administer medications, wound care, treatments, and procedures
Monitor client conditions; report changes to Clinical or Client Services Manager
Case management and coordination
Document observations, interventions, and evaluations using an EMR tablet

🎯 Requirements

Current RN license in DE
Minimum 1 year of clinical (medical/surgical) experience
Graduation from accredited nursing program
Ability to work independently and manage time
Strong interpersonal skills; EMR experience preferred
Prior home care experience preferred but not required
