Follow the designated care plan per patient needs
Make home visits to clients in Dover, Camden, Magnolia
Administer medications, wound care, treatments, and procedures
Monitor client conditions; report changes to the Clinical/Client Services Manager
Document and follow up on doctors' orders
Perform client assessments and coordinate care

🎯 Requirements

Active Delaware RN license.
Minimum 1 year clinical (medical/surgical) experience.
Graduation from accredited nursing program.
Ability to work independently and manage time effectively.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
