Registered Nurse, Home Health

📋 Description

• Perform home health visits, assessments, and admissions.
• Follow designated care plans for adult patients in assigned territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor patient conditions; report changes to Clinical or Client Services Manager.
• Document doctors' orders and care using EMR touchpad.
• Provide case management and coordination as part of a multidisciplinary team.

🎯 Requirements

• Current RN license in Delaware.
• At least 1 year of clinical (medical/surgical) experience.
• Home care experience preferred, but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; EMR experience preferred.
