📋 Description

• Provide home health visits, assessments, and admissions for adults.
• Follow designated care plans per patient needs.
• Medication administration, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the clinical team.
• Document orders and care activities using EMR on a touchpad tablet.
• Coordinate case management and follow-ups with the team.

🎯 Requirements

• Active Delaware RN license.
• Minimum 1 year of recent, verifiable clinical experience.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
