📋 Description

• Follow a designated care plan per patient needs.
• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions; report changes to Clinical or Client Services Manager.
• Follow up with, execute, and document doctors' orders; perform client assessments.
• Case management and coordination; document observations in EMR on a tablet.

🎯 Requirements

• Current NJ RN license.
• Minimum 1 year recent clinical (medical/surgical) experience.
• Graduation from accredited nursing program.
• Prior home care experience strongly preferred.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills with EMR experience; ability to travel to cases.
