📋 Description

• See clients through care needs from hospital discharge to safe home life.
• Deliver 1:1 skilled nursing, meds, wound care, treatments, and comfort care per care plan.
• Perform assessments, educate patients/families, and monitor condition.
• Travel to cases as assigned to provide in-home care.

🎯 Requirements

• Active and current RN license in PA.
• One year of clinical experience strongly preferred.
• Home healthcare experience welcomed but not required; training provided.
• Ability to travel to assigned cases.
• Compassionate, excellent communication, and reliable patient care.
