Acute and post-acute care (CHF, COPD, wound care, IV therapy)
Triage and manage urgent clinical situations independently in the home
Medication reconciliation, teaching, and safety monitoring
Comprehensive wound assessment, care, and documentation
Comfortable making real-time clinical decisions without immediate on-site support

🎯 Requirements

Current nursing license in good standing in NH
Graduation from an accredited nursing program
At least one year of hands-on experience with adults/geriatric patients
