Registered Nurse,-Home Health Visits

📋 Description

• Provide RN home health visits in Lebanon County, PA.
• Follow designated care plans with 1:1 patient focus.
• Perform meds administration, wound care, and procedures per orders.
• Monitor patient conditions and report changes to supervisor.
• Complete documentation on EMR and touch pad tablet.
• Coordinate care and case management with clinical team.

🎯 Requirements

• Current Pennsylvania RN license.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduate from accredited nursing program.
• Ability to work independently and travel to cases as assigned.
• Strong interpersonal and computer skills; EMR experience preferred.
