Registered Nurse-Home Health Visits

📋 Description

• Follow a designated care plan for adult and geriatric patients in Lehigh County.
• Make home visits to clients in assigned geographic territories.
• Administer medications, perform wound care, and other procedures as needed.
• Monitor client conditions and report changes to Clinical/Client Services Manager.
• Document orders, assessments, and interventions accurately for care management.
• Coordinate with the care team to ensure high-quality home health services.

🎯 Requirements

• Current Pennsylvania Registered Nurse license.
• Minimum of one year of recent clinical (medical/surgical) experience.
• Home care experience strongly preferred, but not required.
• Graduate of an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and computer skills; EMR experience preferred.
