• Conducts initial in-person clinical assessments and coordinates service activation, ensuring that

• Actively coordinates the care of the client through communication and written documentation to the

🎯 Requirements

• Valid and active Registered Nurse (RN) license in the State of Connecicut
• Ability to articulate cases through effective case summaries.
• Ability and flexibility to travel (usually by car) within assigned geographic area; valid driver’s
• Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office)
• A passion for providing a high level of customer service, including the ability to show empathy
