Registered Nurse, Per Diem Weekends, Home Health

📋 Description

• Visit clients in their home per care plan
• Case management, care plans, assessments, and wound care
• Progress clients from discharge to achieving home goals
• Educate patients and families on care
• Weekend per diem schedule

🎯 Requirements

• Active RN license
• One year of clinical experience preferred
• Home health experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned
