Make home visits to clients after hours and on weekends.
Administer meds, wound care, treatments, and comfort care.
Monitor client conditions; report changes to the Clinical or Client Services Manager.
Document and follow up on physicians’ orders.
Perform client assessments as needed.
On-call schedule: 5:00 PM–8:30 AM; one week on, one week off.

🎯 Requirements

Current RN license in Vermont and New Hampshire.
Minimum one year of recent clinical (medical/surgical) experience.
Prior home care and/or hospice experience preferred, but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to assigned cases.
