📋 Description

• Provide skilled nursing and rehabilitative care to clients at home as part of a multi-disciplinary
• Conduct comprehensive head-to-toe assessments and identify changes in patient condition.
• Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triaging and independently managing urgent clinical situations in the home.
• Perform medication reconciliation, patient teaching, and safety monitoring.
• Document wound assessments and care using point-of-care technology.

🎯 Requirements

• Current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited nursing program.
